- Physical exam with PPD, Rubella and Varicella (within last year)
- Nurse State License
- Malpractice Insurance (optional, but may be required by some clients)
- Copies of two of the following: driver's license, Social Security card, passport, birth certificate or proof of citizenship
- Resume
- Proof of CPR Training
- Two References
